Eligibility
In order to register we have to validate the following information about you:
- Your ID – We will need to see an original photo ID (picture, name and date of birth)
- Your Proof of Address – Original utility bill or equivalent
- Your eligibility to NHS primary care – if you do not have a valid UK Passport then please ask at reception for advice on eligibility. As a general rule, if you are legally allowed to reside in the UK for more than six months then we would normally register you.
As well as bringing the above originals we would be grateful if you would bring photocopies of each document.
If you are not eligible to register you may be able to be seen as a private patient. Please enquire at reception for details.
How to register
- Please arrive at reception between 14:30 – 15:00 on any weekday except thursday.
- The documentation discussed above will be verified and photocopies kept.
- You will be given two forms to complete for each person being registered. Please fill in all required information
- The receptionist will arrange a suitable date for your New Patient Health Check.
Please note:
Please be aware that you will not normally be able to see a clinician on the day you register.
The Forest Surgery is a popular practice. New registration load can be high and it may take one week or longer for you to be registered on to our computer system.
If you fail to attend your New Patient Health Check you will receive a letter. If you fail a second time you will be removed from the list.
It can sometimes take several months for your records to arrive at the Practice. Therefore when arriving for your check-up, please bring your drug list and whatever information you are able to obtain from your previous GP.
New Patient Health Check
You will normally be seen by a Health Care Assistant who will record your height, weight, blood pressure and other medical details.
PLEASE BRING YOUR MEDICATION, ANY MEDICAL RECORDS YOU HAVE AND A URINE SAMPLE FOR YOUR HEALTH CHECK. |
